A Risk Assessment Form is a form that informs us all the possible risks that may harm the crew. So you have to go though the list and tick all the possible risks that may harm your crew. You then need to decide whether that risk is present and if so, how severe it is, how likely it is and work out the overall risk factor. You can then go into detail about the risk and decided what action you're going to take to prevent the worst from happening. I think that common sense comes into use here but you must always try and imagine what the worst thing that could possibly happen and try and prevent the risk to happen.
